What Are the Requirements for the Medical Card Program in New Jersey?

  • 18 years of age or older (minors will need to submit a birth certificate and documentation of legal guardianship)

  • Faxed-in recommendation from a physician, to your dispensary of choice, for use (that’s what we give you!)

  • Proof of residency such as a New Jersey driver’s license or state-issued ID

  • A phone or electronic device for your telemedicine appointment 

How Much Does it Cost to Get My New Jersey Medical Card?

 

  • $125 for a New Patient Medical Card evaluation

  • $99 for a Renewing Patient Medical Card Evaluation

  • After your doctor’s visit, you’ll turn in an application to the state of New Jersey. It costs $50, or $20 for certain patients with limited income
